前言

  更年期是指婦女從生育期過度到老年期的這一段歲月,是指卵巢功能逐漸萎縮、衰退到完全消失的這段時間,月經將絕未絕,腎氣漸衰的這段時期通常開始於四十五歲至五十歲時左右。月經往往不規則,週期或長或短,經量或多或少甚至停經,這是正常生理現象。

  有不少婦女在此時出現的症候叫更年期綜合徵,所表現的症狀多少不一,因人而異,易受氣候、環境、情緒等因素所影響而時隱時現或時輕時重,嚴重時可影響生活、工作和事業,給患者造成極大痛苦,甚至影響家庭和睦及社會聯繫,如煩躁易怒、轟熱汗出、頭暈目眩、心悸失眠、口乾咽燥、手足心熱、精神疲憊、耳鳴健忘等。這種情況有時遷延數月,有時要延續數年或更久,少數婦女也有超過15年之久,隨著更年期後,患者的各種病症如惡性腫瘤、動脈粥狀硬化、高血壓、骨質疏鬆、功能性子宮出血、老年性陰道炎、尿道炎等的發病率明顯上升。給生活帶來一定的影響,所以知己知彼女人們應該積極瞭解,男士們也要瞭解些並協助她們度過這段時期

  黃帝內經上古天真論:「七七任脈虛,太沖脈衰少,天揆竭,地道不通,故形壞而無子也。」中醫認為婦女四十九歲接近絕經期前後,任脈和沖脈都已衰退,沖任虧損,精血不足,臟腑功能失調,引起各種身心不適症狀;腎氣漸衰月經斷絕,形體衰老,所以就不能再受孕了,也造成如上所述更年期一系列的症狀。

  中醫對更年期的治療採辯證論治的方法。更年期症候群的根本原因在於腎虛,臨床上可分為腎虛肝旺及腎陰陽兩虛二種證型。

一.腎虛肝旺型:腎陰虛,腎水不足,故引起肝失涵養,虛火上升。

症見:月經週期紊亂,量少,色鮮或紫暗,頭暈目眩,煩躁易怒,失眠耳鳴,潮 熱多汗,兩顴升火,有轟熱感,舌質紅,脈細數。

治法:滋腎平肝,知柏地黃湯為主。生地 ,淮山藥 ,山萸肉 ,茯苓 ,澤瀉 ,丹皮 ,知母 ,黃柏 。如症見口苦,肝火偏亢者,加未逍遙散加龍膽草等

二.腎陰腎陽兩虛型:腎陰虛,久而久之陰損及陽,造成腎陰腎陽兩虛。

症見:頭暈目眩,失眠耳鳴,腰膝乏力,手足欠溫,舌質淡,脈沉細。

治法:補腎溫陽,調益沖任。左龜丸或加味二仙湯為主。仙茅 ,仙靈脾 ,當歸 ,巴戟天 ,黃柏 、知母 ,桑寄生 ,杜仲 ,枸杞子 ,菊花

    

  總結本中心多年臨床病例中可以發現到更年期婦女往往因伴隨著隱性鼻竇炎導致提早誘發或延後更年期症候群,甚至遷延不去的現象兩者交相影響困擾著病人且易被忽略,並且由於隱性鼻竇炎的嚴重性、頑固往往易造成更年期婦女情緒變化更大,如對壓力溫度的適應性較差一些很有趣的特點:1.不一定有噴嚏、膿涕、鼻塞、但多半有分泌物倒流到咽喉、咽喉癢痛、輕微咳嗽、咯痰、經常清喉嚨,出現類似梅核氣的症狀。2.咽喉分泌物長時間倒流到消化道後,可引起胃腸功能紊亂有些人則又有食慾不振、腹脹、腹瀉、惡心、嘔吐或便秘。3.有頭痛、頭昏、精神不振、頸肩僵硬等。由於它的隱匿性和複雜性,極易讓病人、醫生忽略且又難以根治,而病情逐漸加重,故我們把它稱隱形殺手以期喚起病人及醫生同道的重視。4.更有一些病友因此又引發大量的過敏反應並出現復雜的抗體原反應症狀使更年期症候群 更顯得復雜離奇。

  台灣地區45歲以上女性有300多萬人,約占總人口數15﹪左右,並以每年3-5﹪的比例成長北美地區也一樣。通常女人一過了45歲,常感覺身體、精神什麼都不對,心理上的更年期較生理上的問題提早發生,體內雌激素逐漸減少,骨質快速流失, 生理上漸漸會有骨密度流失、熱潮紅及盜汗等症狀,有些人更在停經之前,會出現明顯的「失眠」、「疲倦感」、「肌肉痠痛」及「關節痠痛」的發生率最高,其次是有七成六在四十五歲以後常遭遇頭暈、目眩、身體異常發熱及夜間盜汗。未停經者在「感到失落」的發生率,高於已停經者。當發生失落感、脾氣暴躁或睡眠不好等心理及精神症狀時,就必須請求專業人員協助;除看醫生按時服藥以外可選擇與親近的家人談談抒發,多參與社區活動,若情況未獲改善平時可作簡易的靜態腳功運動操,假日可從事輕度戶外運動,保持愉快的心情,如散步、慢跑、跳土風舞等,爬山應以平坦坡度為宜,但不要選過陡的山,如此將可紓解心理上的壓力,飲食以鈣質的補充及五穀、蔬菜為主,還要忌刺激性食物,如:蛋黃、奶油、魷、腦髓、油膩食物、咖啡、大蒜、辣椒、茶葉、高熱量、高膽固醇食物亦應少吃。

United States

In the United States in 2000, there are an estimated 41.75 million women over age 50. About 31.2 million women are over 55 years (compared with 28.7 million in 1990); by the year 2020, the number of US women over 55 is estimated to be 45.9 million.
(See Table 1).

A womans life expectancy is estimated at 79.7 years. Today, a woman who reaches 54 can expect to reach the age of 84.3 years. About two-thirds of the total US population will survive to age 85 or longer. Most women spend one-third to one-half of their lifetime in postmenopause.
 

The North American Menopause Society is often asked (1) how many US women are postmenopausal and (2) how many women are reaching menopause this year. No hard data exist. NAMS provides the following rough estimates, as well as the assumptions that have been used to make these estimates.

  1. Natural (spontaneous) menopause (not premature). The average age of natural menopause in the Western world is approximately 51 years. In the report of the 1990 census from the US Census Bureau, estimates of the population were made in 5-year age groups (See Table 1). The Census Bureau does not provide the exact number of women over age 51. The report projected that 8.85 million women would be age 50-55 on July 1, 2000. One can assume that 50% of 51-year-olds have reached menopause and that percentage would increase to 90% at 55 years of age, allowing the overall percentage over the 5-year span to be approximately 75%. Thus, 75% of 8.85 million is 6.64 million -- the number of women who have reached menopause in this age group. This number is added to 32.85 million, the number of women age 55-plus. Therefore, 39.39 million women will have reached natural menopause (and are spontaneously postmenopausal) as of July 1, 2000. For a rough estimate of the number of women reaching natural menopause during 2000, 6.64 million is divided by 5, for a total of 1.328 million.
     
  2. Premature (spontaneous) menopause. Clinical trial data suggest that the incidence of premature ovarian failure (POF) in US women is about 0.3%, or approximately 146,000 cases in 2000. Although POF is not always synonymous with premature menopause, this figure can be used as a rough estimate for premature spontaneous menopause. However, this is the number of US cases; the annual figure is not known.
     
  3. Surgical menopause. The 1997 National Hospital Discharge Survey (published in 1999) indicates that 481,000 women experienced bilateral oophorectomy in the United States that year. Although statistics are not available for 2000, this figure can be used as a rough estimate for the number of women experiencing surgically induced menopause.
     
  4. Induced menopause from other causes (eg, chemotherapy, pelvic radiation therapy). There are no hard data from which to calculate estimates, so these women are not included.
     

Because of the limitations, these figures can only be used to provide a rough estimate of the number of US women reaching menopause -- either naturally or induced -- in the year 2000:

Canada

Canadian statistics also show an increase in life expectancy for midlife women. In 1922, a 50-year-old woman lived, on average, until age 75. Today, a woman the same age can live until her mid-80s. It has been projected that more than 4 million women (15% of the Canadian population) would be age 50 and older in 2000. Canadian women are, therefore, living at least one-third of their lives in postmenopause.

Worldwide

Globally, there are over 470 million women aged 50-plus, and 30% live to age 80.
